OVERTON COUNTY PHYSICAL THERAPIST FACES FORGERY CHARGES
NASHVILLE – An investigation by Special Agents with the Medicaid Fraud Control Unit of the Tennessee Bureau of Investigation has resulted in the arrest of a Livingston physical therapist on forgery charges.
At the request of 13th District Attorney General Bryant Dunaway, on October 6, 2017, TBI Agents began investigating Dustin Rich on allegations of TennCare fraud. At that time, Rich was employed by a Clay County hospital as a physical therapist. During the course of the investigation, Agents developed information that Rich created false documents and forged the signatures of the patient and/or guardian, which resulted in TennCare being fraudulently billed for those services. The investigation revealed that from January to May 2017, Rich fabricated summary notes of therapy visits he did not complete, and forged the signatures, from his personal residence in Overton County. Rich is no longer employed by the hospital.
On May 21st, the Overton County Grand Jury returned indictments charging Dustin Chase Rich (DOB 04/20/1988) with four counts of Forgery. On Friday, TBI Agents arrested Rich. He was booked into the Overton County Jail and released on his own recognizance.
